I've noticed that categories seem to update very slowly - actually, not at all. For example, I've been working down the Category:Pages with reference errors (a redlink, but still a useful auto-generated category for maintenance purposes), but articles I fixed weeks ago (all numericals and all entries under the letter "A") are still on the list. I can't remember where, but I've noted this elsewhere too in an existing category. And another user also recently commented on those problems, but of course I can't find that citation either.
Still - is it possible that something broke in the last wiki update?

I just noticed that problem myself. What's really weird, though, is that I can see that the articles I've added are listed in the Academic Institutions category while I'm logged in, but if I check the category while logged out they disappear.
Right now I can see 15 articles listed in Category: Academic Institutions while I'm logged in, but while logged out there are only 6.

There definitely seems to be something funky for logged in vs. anyonmous users.
Non-logged-in users will see "cached" versions of pages in many cases. After purging these pages:

Anonymous users now see all 15 pages in the first category and 11 in the second, which is different from the 6/60 items they showed before the purge.
I'm going to look into if there's a setting where the page can be automatically purged when edits to categories are made.

Apparently this is a well-known issue when using the $wgFileCache (which we use to speed up the site for anonymous users):

I've worked around it by ensuring Category: pages are cleared from the file cache once a day.
