History
Leutnant Jaochim (sic, see Notes below) von Heldenberg was a MechWarrior who piloted his Zeus, "Dortmund", in the first live-fire test of Operation ICICLE, a Lyran Commonwealth technology project. The live fire test failed spectacularly, killing all nearby personnel including Von Heldenberg.
It is suspected that a technician made a mistake under fire and failed to correctly attach a coolant hose that would have replenished coolant from a support vehicle. When the Icicle system flushed all coolant from the 'Mech, it thus failed to reacquire new coolant and the 'Mech reached above-critical heat levels that led to an explosion of the on-board ammunition.[1]

He piloted a Zeus, named "Dortmund", outfitted with half the normal complement of heat sinks to facilitate testing.[1]
Notes
- The first name ("Jaochim") is spelled out only one single time in the report about the incident. It may be a misspelling of the German name "Joachim". (There are some other spelling errors in the document, too.)
